Self-injecting testosterone is one of the more anxiety-inducing firsts of TRT — and then after two or three times, it becomes as routine as brushing your teeth. Here's everything you need to know to do it correctly and safely.
What You'll Need
- Testosterone cypionate vial (typically 200mg/mL)
- Syringes with two needle gauges:
- - 18-20g for drawing (thicker needle pulls oil easily)
- - 23-25g for IM injection, OR 27-29g for subcutaneous
- Alcohol swabs
- Gauze pads or cotton balls
- Sharps container (never throw needles in regular trash)
IM vs. Subcutaneous Injection
Intramuscular (IM) — into muscle tissue, faster absorption, traditional standard.
Subcutaneous (SubQ) — into the fat layer under the skin, like an insulin injection. Smaller needles, easier self-administration, generally less painful.
Many TRT physicians now prefer subcutaneous injection for home protocols. The absorption difference (slightly slower) doesn't meaningfully affect outcomes on a regular schedule. For beginners, SubQ is recommended.
Injection Sites
### Subcutaneous (Preferred for Beginners) - Abdomen: 2 inches from the navel on either side. Rotate sides to avoid scar tissue buildup. - Upper outer thigh: Accessible when sitting.
### Intramuscular - Vastus lateralis (outer thigh): Easiest for self-injection. Find the outer front of your thigh, middle third. - Ventrogluteal (hip): Less accessible but considered safest IM site (fewer nerves and blood vessels). - Avoid: The dorsogluteal (upper outer buttocks) for self-injection.
Step-by-Step: Subcutaneous Injection
Before you start: Wash hands thoroughly.
Step 1: Gather supplies Set everything on a clean surface.
Step 2: Prep the vial Wipe the rubber top with an alcohol swab. Let it dry 30 seconds.
Step 3: Draw the medication Attach draw needle to syringe. Pull back plunger to target volume to load air. Insert through rubber stopper, push air in, invert vial, draw to slightly past target volume. Tap syringe to move air bubbles up, push to exact target.
Step 4: Swap to injection needle Replace draw needle with injection needle (27-29g).
Step 5: Prep the injection site Wipe with alcohol swab. Let dry completely — 30-60 seconds.
Step 6: Inject Pinch a fold of skin between thumb and forefinger. Insert needle at a 45-degree angle in one smooth motion. Release the pinch, push plunger slowly over 5-10 seconds.
Step 7: Remove and apply pressure Pull needle out at the same angle. Apply gentle pressure with gauze. Don't rub.
Step 8: Dispose safely Needles and syringes go directly into your sharps container. Never recap a used needle.
Common Beginner Concerns
"I see blood after injecting." Normal — you may have nicked a small capillary. Apply pressure.
"I got a lump or bump." Normal for SubQ — oil collects briefly. Absorbs within 24-48 hours. Rotate injection sites.
"The plunger is stuck." Oil may be cold. Warm the vial in your hands briefly. Ensure needle isn't bent.
"I'm anxious and shaking." This is normal for your first injection. Have everything prepped before beginning. The injection itself takes 30 seconds.
Injection Schedule
For testosterone cypionate: - Twice weekly (Monday/Thursday or similar): Most stable hormone levels — preferred - Once weekly: Simpler but more peaks and troughs between injections
Your physician will specify. Twice weekly generally preferred for minimizing estrogen fluctuations and maintaining consistent mood and energy.
When to Contact Your Physician
- Significant redness, warmth, or swelling lasting more than 48 hours
- Hard nodule that doesn't resolve in a week
- Any systemic symptoms after injection
Most men go from "this is terrifying" to "this takes me 3 minutes" within two weeks.
Frequently Asked Questions
Where do you inject testosterone cypionate?
The most common injection sites are the vastus lateralis (outer thigh), ventrogluteal (hip area), or deltoid (upper arm). Many TRT patients prefer the thigh or ventrogluteal site for self-injection. Subcutaneous injection into abdominal fat is also widely used and easier for beginners.
How often do you inject testosterone cypionate?
Most TRT protocols use twice-weekly injections (Monday/Thursday or similar) to maintain stable testosterone levels. Once-weekly is also common. Twice-weekly reduces the hormone peaks and troughs that can cause mood swings and energy fluctuations.
What needle size should I use for testosterone injections?
For drawing: 18-20g needle. For injecting intramuscularly: 23-25g, 1-1.5 inch. For subcutaneous: 27-29g, 0.5-1 inch. Most clinicians now favor subcutaneous injection with smaller needles for home TRT — less painful and easier to self-administer.
